ABSTRACT

Objectives: Religiosity and spirituality (R/S) have been negatively associated with several mental health problems, including delinquency. The study aimed to investigate the relationship between R/S and interpersonal violence using a systematic review. Methods: We conducted a descriptive systematic review followed by meta-analyses using seven different databases. We included observational studies that assessed the relationship between R/S and different types of interpersonal violence (physical and sexual aggression and domestic violence). Results: A total of 16,599 articles were screened in the databases and, after applying the eligibility criteria, 67 were included in the systematic review and 43 were included in the meta-analysis. The results showed that higher levels of R/S were significantly associated with decreased physical and sexual aggression, but not domestic violence. All selected studies evidenced sufficient methodological quality, with 26.8% being cohort studies. In the subanalyses, the role of R/S was more prevalent among adolescents. Conclusion: There is an inverse relationship between R/S and physical and sexual aggression, suggesting a protective role. However, these results were not observed for domestic violence. Healthcare professionals and managers should be aware of their patients' beliefs when investigating interpersonal violence to create tailored interventions for reducing violent behavior.

ABSTRACT

Objectives: Gene-environment interactions increase the risk of psychosis. The objective of this study was to investigate gene-gene and gene-environment interactions in psychosis, including single nucleotide variants (SNVs) of dopamine-2 receptor (D2R), N-methyl-d-aspartate receptor (NMDAR), and cannabinoid receptor type 1 (CB1R), lifetime cannabis use, and childhood trauma. Methods: Twenty-three SNVs of genes encoding D2R (DRD2: rs1799978, rs7131056, rs6275), NMDAR (GRIN1: rs4880213, rs11146020; GRIN2A: rs1420040, rs11866328; GRIN2B: rs890, rs2098469, rs7298664), and CB1R (CNR1: rs806380, rs806379, rs1049353, rs6454674, rs1535255, rs2023239, rs12720071, rs6928499, rs806374, rs7766029, rs806378, rs10485170, rs9450898) were genotyped in 143 first-episode psychosis patients (FEPp) and 286 community-based controls by Illumina HumanCoreExome-24 BeadChip. Gene-gene and gene-environment associations were assessed using nonparametric Multifactor Dimensionality Reduction software. Results: Single-locus analyses among the 23 SNVs for psychosis and gene-gene interactions were not significant (p > 0.05 for all comparisons); however, both environmental risk factors showed an association with psychosis (p < 0.001). Moreover, gene-environment interactions were significant for an SNV in CNR1 and cannabis use. The best-performing model was the combination of CNR1 rs12720071 and lifetime cannabis use (p < 0.001), suggesting an increased risk of psychosis. Conclusion: Our study supports the hypothesis of gene-environment interactions for psychosis involving T-allele carriers of CNR1 SNVs, childhood trauma, and cannabis use.

ABSTRACT

Resumo Os efeitos da justiça organizacional sobre a saúde dos trabalhadores têm sido investigados em diversas áreas de trabalho. Entretanto, a sistematização da informação disponível sobre os efeitos relacionados às doenças cardiovasculares (DCV) e à diabetes é escassa. O objetivo deste artigo é revisar sistematicamente a associação entre justiça organizacional e DCV e metabólica em trabalhadores adultos. A estratégia de busca incluiu os termos justiça organizacional, doença coronariana, doença cerebrovascular, hipertensão arterial sistêmica, diabetes mellitus e DCV. As bases de dados investigadas foram MEDLINE, EMBASE e LILACS. A qualidade dos estudos foi avaliada pelo instrumento desenvolvido pelo National Institute of Health. Foram identificados 1.959 títulos. Após avaliação, foram selecionados oito estudos. Os indivíduos com percepção de justiça organizacional alta apresentaram menor risco de DCV e metabólica. Baixa justiça organizacional tem repercussões para a saúde cardiovascular e metabólica dos trabalhadores. A elaboração de estratégias para promover justiça organizacional deve ser priorizada para assim mitigar seus impactos para os trabalhadores e as instituições.


Abstract The effects of organizational justice on workers' health have been investigated in several areas of work. However, the systematization of available information on the effects related to cardiovascular diseases (CVD) and diabetes is scarce. This article aims to systematically review the association between organizational justice and CVD and metabolic disease in adult workers. The search strategy included the terms organizational justice, coronary heart disease, cerebrovascular disease, systemic arterial hypertension, diabetes mellitus and CVD. This study investigated the following databases: MEDLINE, EMBASE, and LILACS. The quality of the studies was assessed using the instrument developed by the National Institute of Health. Results: This study identified 1,959 titles. After evaluation, eight studies were selected. Individuals with a high perception of organizational justice showed a lower risk of CVD and metabolic disease, whereas low organizational justice presented repercussions for the cardiovascular and metabolic health of workers. The development of strategies to promote organizational justice must be prioritized and thus mitigate its impacts on workers and institutions.

ABSTRACT

This study aims to evaluate national variation in depression prevalence and in different sociodemographic groups, health behaviors, and macroregions of Brazil from 2013 to 2019. Data were obtained from two nationwide Brazilian surveys - Brazilian National Health Survey 2013 and 2019. Participants aged 18 years or older were included, totaling 60,202 individuals in 2013 and 88,531 in 2019. Depression was evaluated with the Patient Health Questionnaire-9 (PHQ-9). All estimations accounted for the population weights and the complex sampling. The findings showed that during the six years between the two surveys, the prevalence of depression in Brazil increased by 36.7%, going from 7.9% in 2013 to 10.8% in 2019, and this increase is higher among unemployed young adults, aged 18 to 24 years, with the increase in the prevalence of depression almost tripled (3.7 in 2013 and 10.3 in 2019), an increase of 178.4%. Those dwelling in urban areas had a higher increase in the prevalence of depression in the six-year period (39.8%) when compared to residents in rural areas (20.2%). There was an increase in the prevalence of depression from 2013 to 2019 for the worst categories of the three health behaviors included in the study for both men and women: heavy drinking, smoking, and not exercising the recommended level of physical activity. Our results show a significant increase in the prevalence of depression over the six years between the two surveys, mainly among the younger and unemployed men. The country's economic recession during this period may explain these findings.


O estudo objetiva avaliar a variação nacional na prevalência geral da depressão e em diferentes grupos sociodemográficos, comportamentos de saúde e macrorregiões do Brasil entre 2013 e 2019. Os dados foram obtidos de dois inquéritos brasileiros de abrangência nacional, a Pesquisa Nacional de Saúde (PNS) 2013 e a 2019. Entre os participantes com idade de 18 anos ou mais, houve 60.202 indivíduos em 2013 e 88.531 em 2019. A depressão foi avaliada com o Questionário de Saúde do Paciente-9 (PHQ-9). Todas as estimativas levaram em conta os pesos populacionais e a amostragem complexa. Os achados mostraram que durante os seis anos entre as duas edições da PNS, a prevalência de depressão no Brasil aumentou em 36,7%, de 7,9% em 2013 para 10,8% em 2019, com um aumento ainda maior em adultos jovens, no grupo etário de 18 a 24 anos, e naqueles que não estavam trabalhando, onde houve um aumento de quase três vezes na prevalência de depressão (3,7% em 2013 e 10,3% em 2019), ou seja, um aumento de 178,4%. Indivíduos residindo em áreas urbanas tiveram um aumento maior na prevalência de depressão ao longo do período de seis anos (39,8%), em comparação com os residentes em áreas rurais (20,2%). Houve um aumento na prevalência de depressão entre 2013 e 2019 para as piores categorias dos três comportamentos de saúde incluídos no estado, tanto em homens quanto em mulheres: etilismo, tabagismo e sedentarismo. Nossos resultados mostram um aumento significativo na prevalência de depressão nos seis anos entre as duas edições da PNS, principalmente entre homens mais jovens e desempregados. A recessão econômica no Brasil durante o período pode explicar esses achados.


El objetivo de este estudio es evaluar la variación nacional en la prevalencia general de depresión, así como en diferentes grupos sociodemográficos, comportamientos de salud, y macrorregiones del país entre 2013-2019. Los datos se obtuvieron de dos encuestas nacionales brasileñas -Encuesta Nacional de Salud 2013 y 2019-. Los participantes con edades entre los 18 años y con más edad incluyeron a 60 202 personas en 2013 y 88 531 en 2019. La depresión se evaluó mediante el Cuestionario de Salud del Paciente-9 (PHQ-9). Todas las estimaciones justificaron los pesos de la población y el muestreo complejo. Los resultados mostraron que durante seis años entre las dos encuestas, la prevalencia de depresión en Brasil se incrementó en un 36,7%, yendo de un 7,9% en 2013 a un 10,8% en el 2019, y este incremento es mayor entre adultos jóvenes, de 18 a 24 años de edad, que no estaban trabajando, donde casi había un incremento tres veces superior en la prevalencia de depresión (3,7 en 2013 y 10,3 en 2019), un incremento de un 178,4%. Aquellos que vivían en áreas urbanas tenían un incremento mayor en la prevalencia de depresión durante el período de seis años (39,8%), cuando se comparó con los residentes en áreas rurales (20,2%). Hubo un incremento en la prevalencia de depresión entre 2013-2019 para las peores categorías de los tres comportamientos de salud incluidos en el estudio para tanto hombres como mujeres: consumo excesivo de alcohol, fumar, y no practicar deporte al nivel recomendado de actividad física. Nuestros resultados muestran un significado incremento en la prevalencia de depresión durante los seis años entre las dos encuestas, principalmente entre los hombres más jóvenes y desempleados. La recesión económica del país durante este periodo puede explicar estos resultados.

RESUMO Objetivou-se, no presente trabalho, monitorar e identificar precipitações erosivas e as perdas de solo (PSs) por meio de parcelas experimentais instaladas na Sub-Bacia do Córrego do Gigante, calibrar o modelo Water Erosion Prediction Project (WEPP) em sua versão encosta, tendo como base os resultados obtidos nas parcelas, e aplicá-lo na área da sub-bacia, para estimativa das PSs por erosão hídrica. O regime pluvial no período de estudo apresentou elevado grau de erosividade e de concentração, pois 57,3% dos eventos foram erosivos, dos quais, os quatro de maior magnitude, participaram com 47,2% da erosividade anual. As áreas contempladas pelas interações entre classe de solo, declividade (D) e uso de solo, nas parcelas experimentais, totalizaram 174,32 ha (50% da área da sub-bacia), cujos valores obtidos resultaram em PS média ponderada de 0,623 t ha-1 ano-1, e foram referência no ajuste do modelo WEPP na versão encosta, com estimativa de 0,651 t ha-1 ano-1. A aplicação do modelo sobre uma superfície com 93,0% de abrangência da sub-bacia resultou em uma estimativa de 0,802 t ha-1 ano-1 para a taxa de PS. As áreas de solo exposto, embora participando com apenas 7,9% da área da sub-bacia, produziram 85,1% das PSs estimadas, o que, por outro lado, reflete a influência positiva da cobertura vegetal na diminuição da erosão hídrica. Simulando a implantação de cobertura sob a forma de campo/pastagem e/ou reflorestamento, nas áreas de solo descoberto, as taxas de PS na sub-bacia reduziriam significativamente para 0,215 t ha-1 ano-1.


ABSTRACT The objective of the present work was to monitor and identify erosive precipitations and monitor soil losses through experimental plots installed in the Córrego do Gigante sub-basin, to calibrate the Water Erosion Prediction Project (WEPP) model, in its slope version, based on the results obtained in the plots, and apply it to the sub-basin area to estimate soil losses due to water erosion. The rainfall regime during the study period showed high degrees of erosivity and concentration, since 57% of the events were erosive, of which, the four most erosive events corresponded to 47.2% of the annual erosivity. The areas covered by the interactions between soil class, slope, and land use, in the experimental plots, totaled 174.32 ha (50% of the sub-basin area), whose values obtained resulted in a weighted average loss of 0.623 t ha-1 year-1, which were a reference in the adjustment of the WEPP model in the hillside version, with an estimate of 0.651 t ha-1 year-1. The application of the model on a surface with 93.0% coverage of the sub-basin resulted in an estimate of 0.802 t ha-1 year-1 for the soil loss rate. The exposed soil areas, although accounting for only 7.9% of the sub-basin area, produced 85.1% of the estimated soil losses, which, on the other hand, reflects the positive influence of vegetation cover in the decrease of water erosion. Simulating the implementation of cover in the form of field/pasture and/or reforestation in the areas of bare soil significantly reduced the rates of soil losses in the sub-basin to 0.215 t ha-1 year-1.

ABSTRACT

The continuous prospection for molecules that may be useful in the development of new therapeutic agents is a highly relevant issue, mainly because the launch of new drugs on the market does not accompany the emergence of new resistant microorganisms. In this context, this work describes the synthesis of new O-alkylamidoximes and the evaluation of its antifungal activity. The new O-alkylamidoximes were prepared using easy synthetic protocols and tested against three Candida species using the broth microdilution method. The synthesized compounds were obtained in moderate to good yields in high purity and without any observable decomposition. All tested compounds shown moderate antifungal activity against at least one strain of Candida. Despite the moderate activity of the new compounds, this was the first report involving the antifungal activity of O-alkylamidoximes. In view of the low chemotherapy arsenal and the development of fungal strains resistant to traditional antifungal agents, the present study opens new possibilities for the preparation of a new class of more active antifungal agents.

Conclui-se que a maioria das seções possuem funcionários que descumprem as BPF e a agroindústria deve avaliar estes resultados e estudar medidas e práticas educativas e de treinamentos em BPF, para garantir a qualidade e segurança dos produtos elaborados, bem como a prevenção de doenças transmitidas por alimentos, preservando-se a saúde dos consumidores.

It was concluded that the majority of employees involved in non-compliance as GMP and the agribusiness must evaluate these results and study measures and educational practices and training in GMP, to guarantee the quality and safety of the elaborated products, as well as the practices of disease transmission by preserving the health of consumers.

Abstract Hemophagocytic lymphohistiocytosis (HLH) is an uncommon and life-threating condition characterized by major immune activation and massive cytokine production by mononuclear inflammatory cells, due to defects in cytotoxic lymphocyte function. It is even more unusual in renal transplant recipients, in which it is often associated with uncontrolled infection. The mortality is high in HLH and differential diagnosis with sepsis is a challenge. The approach and management depend on the underlying trigger and comorbidities. We report a case of a 50-year-old renal transplant female admitted with fever and malaise 3 months post-transplant and presenting anemia, fever, hypertriglyceridemia, high levels of serum ferritin, and positive CMV antigenemia. Urine was positive for decoy cells and BKV-DNA. Graft biopsy showed CMV nephritis. Both blood and urine cultures where positive for E. coli. Hemophagocytosis was confirmed by bone marrow aspiration. Immunosuppression was reduced, and the patient received high-dose intravenous immunoglobulin and dexamethasone, with complete response after 3 weeks. We highlight the importance of early diagnosis and proper management of a rare and serious condition in a renal transplant patient, which can allow a favorable clinical course and improve survival rate.


Resumo A linfohistiocitose hemofagocítica (LHH) é uma condição incomum e potencialmente fatal, caracterizada por importante ativação imunológica e produção maciça de citocinas por células mononucleares inflamatórias, devido a defeitos na função linfocitária citotóxica. É ainda mais incomum em receptores de transplante renal, nos quais está freqüentemente associada a infecções não controladas. A mortalidade da LHH é alta, e o diagnóstico diferencial com sepse é um desafio. A abordagem e o tratamento dependem do gatilho e das comorbidades subjacentes. Relatamos o caso de uma paciente transplantada renal com 50 anos de idade, admitida com febre e mal-estar 3 meses após o transplante, apresentando anemia, febre, hipertrigliceridemia, níveis elevados de ferritina sérica e antigenemia positiva para CMV. A urina mostrou positividade para células decoy e BKV-DNA. A biopsia do enxerto mostrou nefrite por CMV. Ambas as culturas de sangue e urina foram positivas para E. coli. A hemofagocitose foi confirmada pelo aspirado de medula óssea. A imunossupressão foi reduzida e a paciente recebeu altas doses de imunoglobulina intravenosa e dexametasona, com resposta completa após 3 semanas. Destaca-se a importância do diagnóstico precoce e do manejo adequado de uma condição rara e grave em um paciente transplantado renal, o que pode permitir um curso clínico favorável e melhorar a taxa de sobrevida.

ABSTRACT OBJECTIVE To present the development and validation of the WebAd-Q Questionnaire, a self-report instrument to monitor adherence to antiretroviral therapy in HIV/AIDS centers in Brazil. METHODS The WebAd-Q is an electronic questionnaire that has three questions about the use of antiretrovirals in the last week. It was constructed from interviews and focus groups with 38 patients. Its validity was tested in a study with a sample of 90 adult patients on antiretroviral therapy for at least three months. We used electronic monitoring bottles, pill counting, and self-report interview to compare adherence. The WebAd-Q was answered on the sixtieth day, twice, with at least one hour of interval. The viral load of the patients was obtained from the service records. We have analyzed the agreement between the answers to the WebAd-Q, the associations, and the correlations with viral load and performance compared to other measures of adherence. RESULTS Among the invited patients, 74 (82.2%) answered the WebAd-Q. No difficulties were reported to answer the questionnaire. The average answer time was 5 min 47 sec. The set of three questions of the WebAd-Q obtained agreement of 89.8%, with Kappa of 0.77 (95%CI 0.61-0.94). The non-adherence answers of the WebAd-Q were associated with detectable viral load. We obtained moderate viral load correlations with the non-adherence scale according to the WebAd-Q. For the three questions of the WebAd-Q, patients with non-adherence answers were also reported as less adherent according to the other measures of adherence. CONCLUSIONS The WebAd-Q answered all the issues considered relevant in the validation of questionnaires, was well understood by patients, was associated with viral load, and obtained good agreement and good performance compared to the other measures. The feasibility analysis of its implementation still depends on a national study on its applicability.


RESUMO OBJETIVO Apresentar o desenvolvimento e a validação do Questionário WebAd-Q, um instrumento de autorrelato para monitorar a adesão à terapia antirretroviral em serviços de HIV/Aids no Brasil. MÉTODOS O WebAd-Q é um questionário eletrônico que contém três perguntas sobre a tomada dos antirretrovirais na última semana. Foi construído a partir de entrevistas e grupos focais com 38 pacientes. Sua validade foi testada em estudo com uma amostra de 90 pacientes maiores de 18 anos, sob terapia antirretroviral há pelo menos três meses. Foram utilizadas as seguintes medidas de adesão comparativas: monitoramento eletrônico, contagem de pílulas e entrevista de autorrelato. O WebAd-Q foi respondido no sexagésimo dia por duas vezes, com intervalo mínimo de uma hora. A carga viral dos pacientes foi obtida nos registros do serviço. Analisamos a concordância entre as respostas ao WebAd-Q, associações e correlações com a carga viral e o desempenho em comparação às demais medidas de adesão. RESULTADOS Entre os pacientes convidados, 74 (82,2%) responderam ao WebAd-Q. Não foram relatadas dificuldades em responder ao questionário. O tempo médio de resposta foi de 5 min 47 seg. O conjunto das três questões do WebAd-Q obteve concordância de 89,8%, com Kappa de 0,77 (IC95% 0,61-0,94). As respostas de não adesão do WebAd-Q associaram-se à carga viral detectável. Foram obtidas correlações moderadas da carga viral com escala de não adesão segundo o WebAd-Q. Para as três perguntas do WebAd-Q, pacientes com respostas de não adesão foram também apontados como menos aderentes segundo as demais medidas de adesão. CONCLUSÕES O WebAd-Q atendeu a todos os quesitos considerados relevantes na validação de questionários, foi bem entendido por pacientes, apresentou associação com a carga viral e obteve boa concordância e bom desempenho em comparação a medidas concorrentes. A análise da viabilidade de sua implementação ainda depende de um estudo nacional de aplicabilidade.

ABSTRACT: Objective: The aim of this study was to evaluate the prevalence of malnutrition risk and its association with socioeconomic, behavioral, and health characteristics in the community-dwelling elderly. Methods: A cross-sectional study with individuals aged ≥ 60 years. Nutritional status was evaluated using the Mini Nutritional Assessment. Socioeconomic, behavioral, and health information was also collected from all participants. The association between each variable and the risk of malnutrition was calculated and adjusted using Poisson hierarchical regression. Results: The initial sample consisted of 3,101 elderly people, of whom 28.3% (95%CI 25.3 - 31.4%) were at risk of malnutrition. The multivariate analysis showed that the risk of malnutrition was significantly higher in women without formal education, who did not live with a partner, and identified as black-skinned. The risk of malnutrition was twice as high in individuals with no family income as compared to those who earned at least three minimum wages. Smokers were also more likely to be at risk of malnutrition than individuals who had never smoked. Participants suffering from kidney, respiratory or heart disease were at higher risk of malnutrition than those with no history of such illnesses. Conclusion: These findings could be used to help in the development of health policies and in the establishment of adequate programs aimed at reducing the risk of malnutrition in this population.


RESUMO: Objetivo: O objetivo do trabalho foi avaliar a prevalência do risco de desnutrição e sua associação com fatores socioeconômicos, hábitos comportamentais e condições de saúde em idosos residentes na comunidade. Metodologia: Estudo transversal, realizado com indivíduos com ≥ 60 anos de idade. O estado nutricional foi avaliado pela Mini Avaliação Nutricional e foram obtidas informações socioeconômicas, hábitos comportamentais e de condições de saúde. Para a identificação dos fatores associados ao risco de desnutrição foram realizadas análises brutas e ajustadas por meio da regressão de Poisson, utilizando modelo hierárquico. Resultados: Foram analisados 3.101 idosos e, destes, 28,3% (IC95% 25,3 - 31,4) foram classificados com risco de desnutrição. Na análise multivariada, a prevalência de risco de desnutrição foi significantemente maior entre as mulheres, sem escolaridade, que não vivem com parceiros e cor de pela preta. O risco de desnutrição foi duas vezes maior nos indivíduos sem renda quando comparados aos que recebem acima de três salários mínimos. Os fumantes demonstraram maior probabilidade de risco de desnutrição quando comparados com os não fumantes. Os indivíduos que autorrelataram ter doenças renais, respiratórias e cardíacas apresentaram maior risco de desnutrição que aqueles que não relataram esses problemas. Conclusão: Estes resultados podem servir de subsídio para a formação de políticas de saúde no estabelecimento de programas adequados visando a redução do risco de desnutrição nesta população.

OBJECTIVE: Using magnetic resonance imaging, we aimed to assess the presence of silent brain vascular lesions in a sample of apparently healthy elderly individuals who were recruited from an economically disadvantaged urban region (São Paulo, Brazil). We also wished to investigate whether the findings were associated with worse cognitive performance. METHODS: A sample of 250 elderly subjects (66-75 years) without dementia or neuropsychiatric disorders were recruited from predefined census sectors of an economically disadvantaged area of Sao Paulo and received structural magnetic resonance imaging scans and cognitive testing. A high proportion of individuals had very low levels of education (4 years or less, n=185; 21 with no formal education). RESULTS: The prevalence of at least one silent vascular-related cortical or subcortical lesion was 22.8% (95% confidence interval, 17.7-28.5), and the basal ganglia was the most frequently affected site (63.14% of cases). The subgroup with brain infarcts presented significantly lower levels of education than the subgroup with no brain lesions as well as significantly worse current performance in cognitive test domains, including memory and attention (p<0.002). CONCLUSIONS: Silent brain infarcts were present at a substantially high frequency in our elderly sample from an economically disadvantaged urban region and were significantly more prevalent in subjects with lower levels of education. Covert cerebrovascular disease significantly contributes to cognitive deficits, and in the absence of magnetic resonance imaging data, this cognitive impairment may be considered simply related to ageing. Emphatic attention should be paid to potentially deleterious effects of vascular brain lesions in poorly educated elderly individuals from economically disadvantaged environments.

Abstract Introduction: Human cytomegalovirus is a major cause of morbidity in kidney transplant patients. Objectives: We aimed to study viral replication and serological response in the first months post kidney transplant in patients undergoing universal prophylaxis or preemptive therapy and correlate the findings with the clinical course of Human cytomegalovirus infection. Patients and methods: Independent from the clinical strategy adopted for managing Human cytomegalovirus infection, prophylaxis versus preemptive therapy, the pp65 antigenemia assay and serological response were assessed on the day of transplantation, and then weekly during the first three months of post-transplant. Results: From the 32 transplant recipients, 16 were positive for pp65 antigenemia, with a similar incidence rate in each group. There were no positive results in the first three weeks of monitoring; the positivity rate peaked at week eight. There was a trend for a higher and earlier frequency of positivity in the universal prophylaxis group in which the course of the Human cytomegalovirus infection was also more severe. Despite the differences in clinical picture and in the initial immunosuppressant schedule, the serological response was similar in both groups. Conclusion: Routine monitoring during the first three post-transplant months has a positive impact on the early detection of Human cytomegalovirus viral replication allowing for timely treatment in order to reduce morbidity of the disease. The strategy of universal therapy employing intravenous ganciclovir was associated to a worse clinical course of the Human cytomegalovirus infection suggesting that the use of >10 cells/2 × 105 leukocytes as a cut-off in this setting may be inappropriate.

ABSTRACT OBJECTIVE To assess the relationship between indicators of socioeconomic status and cesarean section in public hospitals that adopt standardized protocols of obstetrical care. METHODS This was a prospective cohort study conducted between May 2005 and January 2006 with 831 pregnant women recruited from 10 public primary care clinics in São Paulo, Brazil. Demographic and clinical characteristics were collected during pregnancy. The three main exposures were schooling, monthly family income per capita, and residential crowding. The main outcome was cesarean section at three public hospitals located in the area. Crude and adjusted risk ratios (RR), with 95% confidence intervals were calculated using Poisson regression with robust variance. We examined the effects of each exposure variable on cesarean section accounting for potential confounders by using four different models: crude, adjusted by mother’s characteristics, by obstetrical complications, and by the other two indicators of socioeconomic status. RESULTS Among the 757 deliveries performed in the public hospitals, 215 (28.4%) were by cesarean section. In the bivariate analysis, cesarean section was associated with higher family income per capita, higher education, lower residential crowding, pregnancy planning, white skin color, having a partner, and advanced maternal age. In the multivariate analysis, after adjustment for covariates, none of the socioeconomic status variables remained associated with cesarean section. CONCLUSIONS In this group, the chance of women undergoing cesarean section was not associated with indicators of socioeconomic status only, but was defined in accordance with major obstetric and clinical conditions.

ABSTRACT OBJECTIVE To evaluate the prevalence of health-related behaviors according to presence and type of depression in Brazilian adults. METHODS Based on a sample of 49,025 adults (18 to 59 years) from the National Survey on Health 2013 (PNS 2013), we estimated the prevalence of health-related behaviors (smoking; passive smoking; frequent or risky alcohol consumption; leisure time physical activity; time watching TV; and eating pattern indicators), according to the presence of depression (minor and major), evaluated by the Patient Health Questionnaire – 9 (PHQ-9), and the report of depressive mood (in up to seven days or more than seven days) over a two-week period. Prevalence ratios were estimated by Poisson regression. RESULTS Evaluated by the PHQ-9 scale, 9.7% of the Brazilian adults had depression and 3.9% presented major depression. About 21.0% reported depressive mood and, in 34.9% of them, that feeling has been present for more than seven days. In individuals with major depression (PHQ-9), higher prevalence was found in almost all unhealthy behaviors analyzed, in particular, smoking (PR = 1.65), passive smoking (PR = 1.55), risk alcohol consumption (PR = 1.72), TV for ≥ 5 hours/day (PR = 2.13), consumption of fat meat (PR = 1.43) and soft drink (PR = 1.42). The prevalence ratios tended to be lower in those with minor depression. Similar results were observed in adults with depressive mood. CONCLUSIONS This study detected relevant association between depression and health behaviors, in particular for smoking and physical activity. The associations found with the PHQ were similar to those observed with the application of a single question about depressive mood. Our results indicate the importance of assessing the presence of depression and the frequency and severity of symptoms when implementing actions for the promotion of healthy behaviors.


RESUMO OBJETIVO Avaliar a prevalência de comportamentos relacionados à saúde segundo a presença e tipo de depressão em adultos brasileiros. MÉTODOS Com base em amostra de 49.025 adultos (18 a 59 anos) da Pesquisa Nacional de Saúde 2013, foram estimadas as prevalências de comportamentos relacionados à saúde (tabagismo, fumo passivo, consumo frequente ou de risco de álcool, atividade física de lazer, horas de TV e indicadores de padrão alimentar), segundo a presença de depressão (menor e maior), avaliada pela escala PHQ-9, e o relato de humor depressivo (em até sete dias ou em mais de sete dias) em um período de duas semanas. Razões de prevalências foram estimadas por meio de regressão de Poisson. RESULTADOS Avaliados pela escala PHQ-9, 9,7% dos adultos brasileiros apresentaram depressão, e 3,9%, depressão maior. Cerca de 21,0% relataram humor depressivo e em 34,9% deles esse sentimento esteve presente por mais de sete dias. Nos indivíduos com depressão maior (PHQ-9), foram constatadas prevalências mais elevadas de quase todos os comportamentos não saudáveis analisados, principalmente tabagismo (RP = 1,65), fumo passivo (RP = 1,55), consumo de risco de álcool (RP = 1,72), horas de TV ≥ 5 h/dia (RP = 2,13), consumo de carne gordurosa (RP = 1,43) e refrigerante (RP = 1,42). As razões de prevalência tenderam a ser menores na depressão menor. Resultados similares foram observados nos adultos com humor depressivo. CONCLUSÕES O estudo detectou relevante associação entre depressão e comportamentos de saúde, em especial, para tabagismo e atividade física. As associações encontradas com o uso do PHQ foram similares às observadas com a aplicação de uma única pergunta sobre humor depressivo. Os resultados do estudo sinalizam a importância da avaliação da presença de depressão e da frequência e gravidade dos sintomas quando da implementação de ações para a promoção de comportamentos saudáveis.

ABSTRACT OBJECTIVE To describe the prevalence of common mental disorders in Brazilian adolescent students, according to geographical macro-regions, school type, sex, and age. METHODS We evaluated 74,589 adolescents who participated in the Cardiovascular Risk Study in Adolescents (ERICA), a cross-sectional, national, school-based study conducted in 2013-2014 in cities with more than 100,000 inhabitants. A self-administered questionnaire and an electronic data collector were employed. The presence of common mental disorders was assessed using the General Health Questionnaire (GHQ-12). We estimated prevalence and 95% confidence intervals of common mental disorders by sex, age, and school type, in Brazil and in the macro-regions, considering the sample design. RESULTS The prevalence of common mental disorders was of 30.0% (95%CI 29.2-30.8), being higher among girls (38.4%; 95%CI 37.1-39.7) when compared to boys (21.6%; 95%CI 20.5-22.8), and among adolescents who were from 15 to 17 years old (33.6%; 95%CI 32.2-35.0) compared to those aged between 12 and 14 years (26.7%; 95%CI 25.8-27.6). The prevalence of common mental disorders increased with age for both sexes, always higher in girls (ranging from 28.1% at 12 years to 44.1% at 17 years) than in boys (ranging from 18.5% at 12 years to 27.7% at 17 years). We did not observe any significant difference by macro-region or school type. Stratified analyses showed higher prevalence of common mental disorders among girls aged from 15 to 17 years of private schools in the North region (53.1; 95%CI 46.8-59.4). CONCLUSIONS The high prevalence of common mental disorders among adolescents and the fact that the symptoms are often vague mean these disorders are not so easily identified by school administrators or even by health services. The results of this study can help the proposition of more specific prevention and control measures, focused on highest risk subgroups.


RESUMO OBJETIVO Descrever a prevalência de transtornos mentais comuns em adolescentes escolares brasileiros, segundo macrorregiões, tipo de escola, sexo e idade. MÉTODOS Foram avaliados 74.589 adolescentes participantes do Estudo de Riscos Cardiovasculares em Adolescentes (ERICA), estudo transversal, nacional, de base escolar, realizado em 2013-2014 em municípios com mais de 100 mil habitantes. Utilizou-se questionário autopreenchível e coletor eletrônico de dados. Presença de transtornos mentais comuns foi avaliada por meio do General Health Questionnaire (GHQ-12). Estimaram-se prevalências e intervalos de confiança de 95% de transtornos mentais comuns por sexo, idade e tipo de escola, no Brasil e nas macrorregiões, considerando o desenho da amostra. RESULTADOS A prevalência de transtornos mentais comuns foi de 30,0% (IC95% 29,2-30,8), sendo mais elevada entre meninas (38,4%; IC95% 37,1-39,7), quando comparadas aos meninos (21,6%; IC95% 20,5-22,8) e entre os adolescentes de 15 a 17 anos (33,6%; IC95% 32,2-35,0), em relação àqueles entre 12 e 14 anos (26,7%; IC95% 25,8-27,6). As prevalências de transtornos mentais comuns aumentaram conforme a idade, para ambos os sexos, sempre maior nas meninas (variando de 28,1% aos 12 anos, até 44,1% aos 17 anos), do que nos meninos (variando de 18,5% aos 12 anos até 27,7% aos 17 anos). Não houve diferença importante por macrorregião ou tipo de escola. Análises estratificadas mostraram maior prevalência de transtornos mentais comuns entre meninas de 15 a 17 anos de escolas privadas da região Norte (53,1; IC95% 46,8-59,4). CONCLUSÕES A elevada prevalência de transtornos mentais comuns entre os adolescentes e o fato de os sintomas serem muitas vezes vagos fazem com que esses transtornos sejam pouco identificados por gestores escolares ou mesmo serviços de saúde. Os resultados deste estudo podem ajudar na proposição de medidas de prevenção e controle mais específicas e voltadas para os subgrupos sob maior risco.

RESUMO: Objetivo: Descrever a prevalência do autorrelato de diagnóstico médico prévio de depressão na população adulta brasileira (18 anos ou mais) segundo fatores sociodemográficos. Métodos: Foram utilizados dados da Pesquisa Nacional de Saúde 2013, inquérito de base populacional. Foi investigado o autorrelato do diagnóstico médico prévio de depressão, recebido em algum momento da vida. Foram calculadas as prevalências e seus respectivos intervalos de confiança de 95% (IC95%), estratificados por sexo, faixa etária, escolaridade, raça/cor da pele, para Brasil, local de residência, grandes regiões, unidades federativas e capitais. Resultados: A prevalência de autorrelato de diagnóstico de depressão em adultos no Brasil foi de 7,6% (IC95% 7,2 - 8,1), sendo maior em mulheres (10,9%; IC95% 10,3 - 11,6) e em pessoas entre 60 e 64 anos (11,1%; IC95% 9,1 - 13,1). Ainda, as maiores prevalências foram em indivíduos sem instrução ou com fundamental incompleto, 8,6% (IC95% 7,9 - 9,3), para aqueles com ensino superior completo, 8,7% (IC95% 7,5 - 9,9); e para aqueles que se autodeclararam brancos (9,0%; IC95% 8,3 - 9,6). Por local de residência, esse autorrelato foi maior em indivíduos residentes na região urbana (8,0%; IC95% 7,5 - 8,4) e na região Sul (12,6%; IC95% 11,2 - 13,9). Conclusão: A análise deste estudo revela a importância de conhecer o acesso ao diagnóstico de depressão no Brasil. É necessário aprimorar o acesso aos serviços de saúde com qualidade em todo o território nacional para abranger as populações mais desfavorecidas. Reduzir as disparidades no acesso aos serviços de saúde é fundamental para garantir que direitos sociais sejam equânimes e universais.


ABSTRACT: Objective: To describe the prevalence of self-reported previous medical diagnosis of depression in the adult (18 years or older), Brazilian population according to sociodemographic factors. Methods: Data from the 2013 National Health Survey, a population-based survey, were used. The self-reported previous medical diagnosis of depression, received at some point during the patient's life, was investigated. Prevalence and their respective confidence intervals of 95% (CI 95%) were calculated, stratified by gender, age group, education level, race/skin color, for Brazil, place of residence, major regions, federative units and capitals. Results: The prevalence of self-reported diagnosis of depression in adults in Brazili was of 7.6% (95%CI 7.2 - 8.1), being higher in women (10.9%; 95%CI 10.3 - 11.6) and among people between 60 and 64 years old (11.1%; 95%CI 9.1 - 13.1). Furthermore, the highest prevalence was among individuals with no formal education or those with incomplete primary education 8.6% (95%CI 7.9 - 9.3), for those with complete tertiary education 8.7% (95%CI 7.5 - 9,9); and for those who declared themselves as white (9.0%; 95%CI 8.3 - 9.6). For place of residence, the self-report was higher in individuals living in urban areas (8.0%; 95%CI 7.5 - 8.4) and in the South (12.6%; 95%CI 11.2 - 13.9). Conclusion: The analysis shows the importance of understanding the access to the diagnosis of depression in Brazil. It is necessary to improve access to quality health services throughout the country to include the underprivileged population. Reducing disparities in access to health services is crucial to ensuring that social rights are universal and equal.

ABSTRACT

BK polyomavirus (BKPyV) is a causal agent of nephropathy, ureteral stenosis and hemorrhagic cystitis in kidney transplant recipients, and is considered an important emerging disease in transplantation. Regular screening for BKPyV reactivation mainly during the first 2 years posttransplant, with subsequent pre-emptive reduction of immunosuppression is considered the best option to avoid disease progression, since successful clearance or reduction of viremia is achieved in the vast majority of patients within 6 months. The use of drugs with antiviral properties for patients with persistent viremia has been attempted despite unclear benefits. Clinical manifestations of BKPyV nephropathy, current strategies for diagnosis and monitoring of BKPyV infection, management of immunosuppressive regimen after detection of BKPyV reactivation and the use of antiviral drugs are discussed in this review.


BK Poliomavírus (BKPyV) é um agente causal de nefropatia, estenose ureteral e cistite hemorrágica em receptores de transplante renal, sendo considerado uma importante doença emergente na transplantação. Rastreamento regular para reativação do BKPyV, principalmente nos dois primeiros anos pós-transplante, com subsequente redução preemptiva da imunossupressão é considerada a melhor conduta para evitar a progressão da doença, já que a eliminação ou redução da viremia é alcançada na grande maioria dos pacientes dentro de 6 meses. O uso de drogas com propriedades antivirais para os pacientes com viremia persistente tem sido tentado, embora sem benefícios claros. As manifestações clínicas da nefropatia por BKPyV, as estratégias para o diagnóstico e monitoramento da infecção por BKPyV, o manejo do regime de imunossupressão após a detecção da reativação do BKPyV e o uso de drogas antivirais são discutidas nesta revisão.
